Joint Commission International Accreditation Standards for Home Care, 2nd Edition, provides the basis for accreditation of home care organizations throughout the world. Joint Commission International (JCI) standards define the performance expectations, structures, and functions that must be in place for a home care organization to be accredited by JCI. 

The standards are divided into two main sections: 1) patient-centered care and 2) home care organization management.

Within each section, standards are separated into chapters. Each standard includes an intent and measurable elements. The intent describes the rationale for the standard and provides examples (when applicable) on the application of the standards. The measurable elements are the specific requirements of the standard. In addition to the standards chapters, an introduction, the eligibility requirements, the Accreditation Participation Requirements (APR), a summary of key accreditation policies, a glossary, and an index are included. 

Highlights of Changes in the Standards Manual

  • Chapter titles and concepts have been reorganized to be consistent with other JCI accreditation manuals, such as the Hospital 7th Edition
  • Introduced requirements for improving the safety of reporting critical results, and handover communication
  • Developed and implemented a process to improve the safety of look-alike/sound-alike medications
  • Developed and implemented a process to identify and address health care-associated infections
  • Increased focus on improving the continuum of care
  • Increased focus on patient centered care
  • Detailed explanations added to the Medication Management and Use (MMU) chapter to help organizations understand what standards and measurable elements apply depending on what type of pharmaceutical services are provided
  • Detailed explanations added to the Facility Management and Safety (FMS) chapter to help organizations understand what standards and measurable elements apply to the home care facility and the patient home environment
